Self-Declaration Form (Air Suvidha 2.0 portal) for passengers arriving in India

As per the travel advisory issued by the Directorate General of Health Services (DGHS),  Ministry of Health & Family Welfare, Government of India, in view of the Ebola outbreak, all passengers (Indian and foreign nationals) arriving in India are required to fill the Self Declaration Form mandatorily on the Air Suvidha 2.0 portal at https://airsuvidha.civilaviation.gov.in before boarding the flight. The portal enables international-arriving passengers to submit a mandatory online Health Self-Declaration covering their 21-day travel history, exposure history and related symptoms.

The form can be completed 24 hours in advance before arrival and should be filled out accurately for swift clearance on arrival. Passengers will need to carry and present the downloaded Self Declaration Form at the International Travel Health Desk or Immigration Counter as required.

Passengers who are suspected / confirmed to have Ebola must immediately report to the Airport Health Officer / Health Desk before immigration clearance. Any traveller developing Ebola symptoms within 21 days of arrival should immediately seek medical care and inform healthcare authorities about their travel history.
