Passenger Medical Clearance Form (MEDA)
Some passengers need special medical consideration when they travel. This may involve clearance for fitness to fly after a recent illness, medical procedure or surgery; if you have an existing medical condition where there is reasonable doubt that you can complete the flight safely without requiring extraordinary medical assistance during the flight; a request for use of a passenger’s own medical equipment during the flight or a need for in-flight oxygen. The purpose of the MEDA (Passenger Medical Clearance) form is for Cathay Pacific, in conjunction with your doctor, to determine that you are able to complete your journey safely and whether any special assistance or medical services are required.
If you are requested to submit a MEDA form, please ensure that both Part 1 and Part 2 are completed in full. Part 1 is to be completed by the passenger and Part 2 by the treating doctor. The completed forms should be submitted to your local Cathay Pacific Reservation office as early as possible, preferably no less than 48 hours prior to your scheduled flight departure time.
